A responsible adult must sign the rental agreement.

Late returns may incur additional charges.

3. Payment Terms

Full payment or agreed deposit required before release.

Accepted payments: Cash, GCash, Bank Transfer, or other approved methods.

4. Security Deposit

A refundable deposit may be required depending on the equipment.

Deposit will be returned after inspection, provided no damage or missing parts.

5. Equipment Care and Use

The renter agrees to:

Use equipment only for its intended medical purpose.

Keep equipment clean and in good condition.

Not modify or repair equipment without permission.

6. Damage or Loss

Renter is responsible for any damage, loss, or missing parts.

Repair or replacement costs will be charged if necessary.

7. Maintenance and Support

Report any malfunction immediately.

Do not attempt to fix the equipment yourself.

8. Cancellation Policy

Cancellation must be made before the scheduled delivery or pickup.

Deposits may be non-refundable for late cancellations.

9. Sanitation and Safety

All equipment is cleaned and sanitized before release.

Customers are encouraged to maintain cleanliness during use.

10. Liability

The company is not liable for injuries resulting from misuse of equipment.

Proper usage instructions must be followed at all times.
